Patient Portal
The patient portal is a secure, online website that gives patients convenient, 24-hour access to personal health information including a summary of your medical visit and test results. Patients must register for the portal to gain access and registration instructions are provided below.

BRIDGE PATIENT PORTAL REGISTRATION INSTRUCTIONS

- When you arrive for your appointment, you will be asked to provide an email address that will be used to register you for the patient portal. An email address is required to access the portal.
- Within 24 hours of your appointment, you will receive a welcome email from DAK containing a link and instructions on how to set up your Patient Portal account. The link will expire 48 hours after it has been sent
- The website for the patient portal is https://dak.bridgepatientportal.com.
- Once you set up your account and create a password, all future appointments and documents will be available to view. Please note, if you would like access to any previous appointment summaries or documents, you will need to call (859) 263-4444 ext. 2501 and request those documents.
For minors: Children under the age of 18 will not be sent a welcome email after their appointment. If you would like access to your child’s patient portal account, please contact (859) 263-4444 ext. 2501 for assistance. All other portal questions, or assistance with access, can be directed to the number above as well.
