Appointments
All appointments are scheduled through our main location at (859) 263-4444. For your convenience we now offer appointment reminders via phone calls, texts, and email. Please be sure to have your insurance information available when scheduling an appointment. We also ask that you plan to arrive at least 15 minutes prior to your appointment to complete appropriate paperwork.


Medical Records Requests
To request a copy of your medical records you will need to complete the Release of Information Form and return to us. You can access this form by clicking the following link: Medical Records Release form. If you cannot print this form please contact us and we’ll forward a copy to you. Once the form is complete you can mail or fax this to our main office location. The address and fax number are provided below. We will process your request within 30 days of receiving your request.

School/Work Excuses
If you need a school or work excuse, please request this at check-out the day of your appointment.


Prescription Refill Requests
You may request a prescription refill by contacting our main office number at (859) 263-4444 during business hours. (Prescription refill requests will not be handled after hours.) In order to provide you with timely service please have the name of your healthcare provider available along with prescription and pharmacy information.

Also, please be advised our clinical team is assisting with patient care throughout the day, therefore if you are unable to speak with a clinical assistant directly you will be routed to the clinical staff’s voice mail where you can provide prescription refill information. A clinical assistant will return your call by the end of the business day or within 24 hours. All refill requests are reviewed and approved by the healthcare provider and occasionally it may be necessary for you to return to the office for re-evaluation of your condition before a prescription refill request is authorized.


Pathology Results
Our office includes an onsite dermatopathology laboratory which is equipped to perform biopsies and report pathology results as authorized by your provider. You should receive notification of pathology results by logging into the Patient Portal within two weeks of your visit. You may check the portal for the results at that time. If you have trouble logging into the portal, please call the office. Results will only be given to the patient directly therefore if you would like them given to another person, you must inform the clinical assistant before you leave the office.
